Does it need the plugin?
Yes. Site Health, the database and the disk can only be read from inside WordPress, by the Relvato plugin (1.36.0 or later). It reads; it changes nothing.
My host says my old PHP is patched. Why does Relvato flag it?
Some hosts backport security fixes to PHP versions that PHP itself no longer supports. Relvato can't see that from inside the site, so it flags the official end-of-life date and the finding says to check with your host. If they confirm, ignore the finding and it stays quiet.
